Note
- Program book and Abstract CD-R are included in the registration fee.
- Online registration will be closed on February 28, 2015. Please register On-site after this date.
- All the contributors are required to register by February. 28, 2015.
- Refunds for registration and banquet cannot be made at any reason.

- Banquet fee is NOT included in Registration fee. If you would like to attend this event, please pay extra fees to attend.
II. Application Procedure
Please click “On-line Application Form” button at the bottom of this page.
Please complete the application form and submit it to us.
Upon your submission of the application form, "Pre-Confirmation" will be sent to you automatically by E-mail. In the event of NOT receiving Pre-Confirmation, please contact us by E-mail (isplasma@yssf.jp).
For changing your data, cancellation or additional requests, please click the "Change or Cancellation" button at the bottom of this page.
III. Deadline
Online Application page is to be closed on February 28, 2015.
All the contributors are required to register by this deadline.
Please register on-site after this date.
IV. Confirmation
No later than 2-3 weeks prior the Symposium, you will receive "Final-Confirmation" by E-mail.
"Final-Confirmation" must be retained and presented to the registration desk at the Conference venue.
V. Payment
Please note that payment must be made by credit card or bank transfer.
We are sorry but, personal checks are not acceptable.

Your personal data in the application form are secured by SSL (Secure Socket Layer) System.
We will complete your credit card authorization as soon as you finished your registration procedure.
